Before we dive into what direct access physical therapy is, let’s first define the profession of physical therapy (PT) itself.
So… what is PT? The dictionary defines PT as follows:
“Physical therapy, also known as physiotherapy, is a healthcare profession, as well as the care provided by physical therapists who promote, maintain, or restore health through patient education, physical intervention, disease prevention, and health promotion.”
That all sounds great, but what does it really mean?
PTs work with a wide variety of populations and conditions, including postoperative injuries, sprains, strains, general pain, Parkinson’s disease, post-stroke rehabilitation, vestibular conditions, pulmonary and cardiac issues, fall-risk patients, athletes, scoliosis, and more. We treat many different types of patients across multiple settings—hospitals, outpatient clinics, and schools. We also provide care for injury prevention! I always say… everyone and anyone could benefit from PT.
Physical therapists use a combination of therapeutic exercises, manual therapy techniques (such as joint mobilization and massage), electrical stimulation (like TENS), aquatic therapy, balance training, and patient education to improve mobility, manage pain, and restore function based on the patient’s condition.
Now, What Is Direct Access PT?
Direct Access is a law enacted in 2014 that allows patients to see a physical therapist without a physician’s prescription for up to 21 days or 10 treatment visits. However, some insurance providers may still require a referral. This law officially took effect on January 1, 2015, making Michigan a Direct Access state.
Why Does This Matter to You?
Most people appreciate saving both time and money, and that’s exactly what Direct Access PT allows you to do!
A 2014 systematic review by Ojha et al. analyzed eight research studies and found that:
“Physical therapy by way of direct access may contain healthcare costs and promote high-quality healthcare.”

✅ Fewer PT visits
✅ Reduced imaging orders
✅ Decreased reliance on medications
✅ Fewer additional non-PT appointments
Bottom Line: Direct Access Saves You Time & Money
